Summary:
The PASRR Specialized Services Staff/ MPW Assessor/Waiver CM is responsible for conducting Initial MPW Assessments, and Reassessments for outside CM agencies per the State Contract for our 7 county coverage area.
This position is also responsible for providing 1:
1 specialized services to those who live in nursing facilities and have been approved by the State to receive these services.
May carry a small caseload for those participants receiving waiver services.
Minimum
Qualifications:

High School Diploma/GED
1 year experience working with individuals with intellectual and/or developmental disabilities.
